When adding a service type, you see duplicate entries (e.g. Access, Access, Ipswitch Mail 6, Ipswich Mail 6).

Checking the error logs, you may see errors similar to these:

AND LimitPropertyId = (SELECT LimitPropertyId FROM LimitProperty WHERE LimitPropertyName = 'MaxPHPInstals')
(Subquery returned more than 1 value. This is not permitted when the subquery follows =, !=, <, <= , >, >= or when the subquery is used as an expression.)
The key part is "Subquery returned more than one value". There should only be one value for each of these.

This happens if the database set up script has been run twice. Doing this will cause serious issues in Helm. If you have just set up the database you need to drop the database and rebuild it again.
