There are account attributes configured in PBA. When following the "Create Account and Place Order" wizard, the step "Account Attributes" is not listed. The attributes are indeed asked (correct screen shown), but as if it happens on the step 3 "Control Panel Settings":



The issue is identified as a product bug with id #PBA-54139 ("Missing Account Attributes step in wizard Create Account And Place Order").
