Upon removal a customer account that has domains / subscriptions / service users etc, everything is removed from the OA database except for the domains.

Also the issue can manifest itself in a different way: an attempt to remove a subdomain from CCP can fail with the following error:

Duplicate key violates unique constraint 'domains_uni0' while executing ' UPDATE domains SET deleted='y' WHERE domain_id = ?'.

There was a subdomain with same name that was canceled a long ago under the same customer with a different domain_id. One of the domain status was deleted = 'y'. OA did not perform validation while adding a second subdomain with the same name - this issue has been reported as POA-92217.

Domains remain in the domains table after removal, though its mode parameter is changed to d (deleted). This usually leads to a domain record duplication in database, which, in its turn, causes the issue POA-92217 to occur.

POA-92217 was fixed in OA 7.0. To resolve the issue, please upgrade to at least 7.0. To workaround the issue, it is necessary to remove the records with mode = d from domains table manually. Please contact CloudBlue technical support to perform that operation.

WARNING Never perform database operations yourself, however obvious and simple they may seem.
